You may print out the blank questionnaire and answer the questions by hand, or, if it is writeable, you can type your answers directly on the form by clicking on the blank lines or boxes, and print the completed form.

Check boxes ( ) are clickable, and will check themselves after being clicked. To move from line to line or box to box, you can click in the desired area, or hit tab to move the cursor automatically to the next area. Some areas may require multiple lines of typing to continue onto another line, simply hit enter or return.
